Q – Who must provide SBA Form 912, Statement of Personal History? Also, who must provide INS Documents for Verification?
A - Any officer and any one who owns 20 percent or more of the applicant business (OC) and/or real estate holding company (EPC) must complete the form. This form is essential in determining the "character" of management of the OC or EPC. FFCFC staff has years of experience in assisting customers complete and process this form with SBA.
For INS verification, the same applies as above, but also "key" management must provide current/valid INS documents that will must be verified. Again, FFCFC staff have long-standing relationship with INS and can help expedite this verification process.
